ජාතික ඖෂධ නියාමන අධිකාරියේ (NMRA) ප්රතිගාමී වැඩ සිදුවෙනවා..! සිතා මතා කඩාකප්පල්කාරී ක්රියා කරනවා..! 'ශ්රී ලංකා වෛද්ය සංගමයෙන්' ජනාධිපතිට කණ කිස්කන ලිපියක්..!
(ලංකා ඊ නිව්ස් -2023.ජුනි.30, ප.ව.7.10) රටේ පවතින ඖෂධ හිඟයට සහ ප්රමිතියෙන් තොර බාල බෙහෙත් ගෙන්වීමට අදාලව ශ්රී ලංකාවේ ජාතික ඖෂධ නියාමන අධිකාරියේ (NMRA) බොහෝ ප්රතිගාමී වැඩ සිදුවන බවත් එහි නියමිත වැඩකටයුතු කඩාකප්පල් කිරීමට බලධාරීන්ගෙන් සමහරෙකු හිතාමතාම උත්සාහ කරන බවත් ඒ සඳහා කාරණා 4 ක් ගෙන හැර පාමින් ශ්රී ලංකාවේ වෛද්ය වෘත්තිකයන්ගේ උත්තරීතර ආයතනය වන 'ශ්රී ලංකා වෛද්ය සංගමය' ජනාධිපති රනිල් වික්රමසිංහ වෙත ඇස් ඇරවෙන ලිපියක් පසුගිය ජුනි 26 දා යවා තිබේ. ඉංග්රීසි බසින් යවා ඇති ඒ ලිපිය අවසානයේ දැක්වෙන අතර එහි දල සිංහල පරිවර්තනයක් මෙහි දැක්වේ.

ශ්රී ලංකාවේ ජාතික ඖෂධවල වත්මන් අර්බුදය සහ ඖෂධ නියාමන අධිකාරිය (NMRA)
ශ්රී ලංකාවේ වෛද්ය වෘත්තිකයන්ගේ උත්තරීතර ආයතනය වන ශ්රී ලංකා වෛද්ය සංගමය, ශ්රී ලංකාවේ ඖෂධවලට අදාළ වත්මන් අර්බුදය සහ NMRA හි මෑතකාලීන වර්ධනයන් පිළිබඳව අපගේ දැඩි කනස්සල්ල ප්රකාශ කිරීමට කැමැත්තක් දක්වයි. මෙය 2023 ජනවාරි 16 වැනි දින “සෞඛ්ය සේවා පද්ධතිය සහ ශ්රී ලංකාවේ පුරවැසියන් වෙනුවෙන් අභියාචනයක්” යන මාතෘකාවෙන් ඔබ වෙත අප කළ සන්නිවේදනයට තවදුරටත් වේ. වඩාත්ම අවාසනාවන්ත ලෙස, අපට වර්තමාන කාලය දක්වා ඒ පිළිබඳව ප්රතිචාරයක් හෝ පසු විපරම් ක්රියාමාර්ගයක් නොතිබුණි.
පාර්ලිමේන්තුවේ වෙනම පනතක් (2015 අංක 5 දරන NMRA පනත) මගින් NMRA නියාමන අධිකාරිය ලෙස ස්ථාපිත කිරීම හේතුවෙන් බොහෝ අඩුපාඩු සහිතව වුවද රට තුළ ඖෂධ ඇගයීම සහ ලියාපදිංචිය වැඩිදියුණු විය. කෙසේ වෙතත්, අපි දැන් අත්යවශ්ය බොහෝ ඖෂධ හිඟයක් අත්විඳිමින් සිටිමු, ආර්ථික අර්බුදය විසින් වේගවත් කරන ලද සහ වැරදි කළමනාකරණයෙන් තත්ත්වය උග්ර වී ඇත.
අත්යාවශ්ය ඖෂධ සහ ශල්ය ද්රව්ය සැලකිය යුතු ප්රමාණයක් රටේ බොහෝ රෝහල්වල ප්රමාණවත් ප්රමාණයකින් නොමැත. තවද මෙම ඖෂධ හිඟය මඟහරවා ගැනීම සඳහා නිසි ඇගයීමකින් සහ ලියාපදිංචියකින් තොරව කඩිමුඩියේ ආනයනය කිරීම හේතුවෙන් දුර්වල තත්ත්වයේ ඖෂධ සහ ගුණාත්මක අසාර්ථකවීම් කිහිපයක් වාර්තා වේ.
මෙම පසුබිම තුළ, තත්ත්වය නිවැරදි කිරීම සහ වැඩිදියුණු කිරීම වෙනුවට, පහත ලැයිස්තුගත කර ඇති පරිදි NMRA හි බොහෝ ප්රතිගාමී පියවරයන් සිදුවෙමින් පවතින බව අපි සටහන් කරමු. මෙය තත්ත්වය තවත් නරක අතට හැරෙනු ඇති අතර එමඟින් රෝගීන්ගේ ආරක්ෂාව අනතුරේ.
ඖෂධවේදය පිළිබඳ මහාචාර්යවරයකු මණ්ඩලයට පත් කර නෑ..
1. ශල්ය වෛද්ය විද්යාලය සහ ප්රසව හා නාරිවේද වෛද්ය විද්යාලය මගින් දැනටමත් නම් කර ඇති ඖෂධවේදය පිළිබඳ මහාචාර්යවරයකු, වෛද්ය විශේෂඥයන් සහ සුදුසුකම් ලත් ඖෂධවේදියකු / ජ්යෙෂ්ඨ කථිකාචාර්යවරයකු හෝ ඖෂධවේදය පිළිබඳ මහාචාර්යවරයකු මණ්ඩලයට පත් කිරීමට ගරු සෞඛ්ය අමාත්යවරයා මෙතෙක් අපොහොසත් වී ඇත. NMRA පනතේ විධිවිධාන අනුව, තාක්ෂණික විශේෂඥතාව සැපයීම සඳහා මෙය අත්යාවශ්ය ය.
මිලකරණ කමිටුව අක්රීයයි..
2.ඖෂධවල සිල්ලර මිල අනුමත කිරීමේ සාකච්ඡා සඳහා වගකිව යුතු NMRA හි මිලකරණ කමිටුව තවදුරටත් ක්රියාත්මක නොවේ. ක්රියාකාරී සහ ක්රියාකාරී මිල කමිටුවක් නොමැතිව අලුතින් ලියාපදිංචි වූ ඖෂධවල මිල තීරණය කරන්නේ කෙසේදැයි අපට තේරුම් ගැනීමට නොහැකි වී ඇත.
අසාර්ථකත්වයන් ඇගැයීමේ කමිටුව පත් කර නෑ..
3. තත්ත්වයේ අසාර්ථකත්වයන් ඇගයීම සඳහා වූ කමිටුව ද නැවත පත් කර නොමැති අතර, එමගින් තත්ත්වය වැඩිදියුණු කිරීමට කිසිදු ක්රියාමාර්ගයක් ගැනීම වැළැක්වීය. පසුගිය කාලයේ රෝහල් ගත කළ ඇතැම් මරණවලට ප්රමිතියෙන් තොර ඖෂධ නිසා සිදුවූ මරණ බවට දැනටමත් බරපතළ චෝදනා එල්ල වී තිබේ.
ජ්යෙෂ්ඨ පළපුරුදු ඖෂධවේදීන් රැසක් හදිසියේම මාරු කරලා..
4. NMRA හි ඖෂධ ඇගයීම් කටයුතුවල නිරතව සිටින ජ්යෙෂ්ඨ පළපුරුදු ඖෂධවේදීන් රැසක් හදිසියේම සෞඛ්ය අමාත්යාංශය වෙත මාරුකර යවා ඇති අතර, ඖෂධවේදීන්ගේ ඇගයීම් සඳහා ප්රමාණවත් නොවන ඖෂධවේදීන් සංඛ්යාවක් ඉතිරි කර ඇත. ලියාපදිංචියට පෙර ඖෂධවල තත්ත්ව තක්සේරුවට මෙය සෘජුවම බලපෑ හැකිය.
සිතා මතා කඩා කප්පල් කරනවා.. බරපතල හානි ගෙන දෙනවා..
ඉහත නියමයන් සැලකිල්ලට ගනිමින්, රටට සපයනු ලබන ඖෂධ ලියාපදිංචි කිරීමේ ක්රියාවලියට සහ තත්ත්ව පාලනයට බලපෑ හැකි NMRA හි වත්මන් වර්ධනයන් පිළිබඳව අප දැඩි කනස්සල්ලට, පීඩාවට සහ සැලකිය යුතු කැළඹීමට පත්වන බව ඔබට ඉතා ඕනෑකමින් දැනුම් දීමට කැමැත්තෙමු. NMRA හි වැඩකටයුතු කඩාකප්පල් කිරීමට බලධාරීන්ගෙන් සමහරෙකු හිතාමතාම උත්සාහ කරන බව අපි අවංකවම විශ්වාස කරමු. එහි ප්රතිඵලයක් ලෙස වෛද්ය වෘත්තිකයන් මෙන්ම සාමාන්ය ජනතාවද ඖෂධවල ගුණාත්මක භාවය සහ ආරක්ෂාව පිළිබඳ විශ්වාසය ගිලිහී යමින් සිටින අතර, එය බරපතල හානිකර බලපෑම් ඇති කරනු ඇත. මෙය විශේෂයෙන්ම එසේ වන්නේ NMRA හි සභාපතිවරයා මාධ්ය සමඟ කළ සාකච්ඡාවලදී දැක්වූ අශික්ෂිත හා නිස්සාර ආකල්පය සැලකිල්ලට ගනිමිනි.
එබැවින්, තවදුරටත් ප්රමාදයකින් තොරව කඩිනම් පියවර ගන්නා ලෙස අපි ඔබෙන් ඉල්ලා සිටිමු.
මෙම කාරණය සම්බන්ධයෙන් ඔබගේ හදිසි මැදිහත්වීම පිළිබඳව අපි ඔබට කල්තියාම ස්තූතිවන්ත වන අතර, එම කාරණය සම්බන්ධයෙන් වැඩිදුර සාකච්ඡා සඳහා ඔබ සමඟ මුහුණට මුහුණ රැස්වීමක් පැවැත්වීමට අපි කැමැත්තෙමු.

Re: The Current Crisis of Pharmaceuticals in Sri Lanka and the National Medicines Regulatory Authority (NMRA)
Sri Lanka Medical Association, the apex body of medical professionals in Sri Lanka, wishes to express our deep concern regarding the current crisis pertaining to the pharmaceuticals in Sri Lanka and the recent developments in the NMRA. This is further to our communication to you dated 16th January 2023, titled “An appeal on behalf of the healthcare system and the citizens of Sri Lanka”. Most unfortunately, we have not had a response or any follow-up action on it, up to the present time.
The establishment of the NMRA as the Regulatory Authority by a separate Act of Parliament (NMRA Act No 5 of 2015), resulted in improvements in the evaluation and registration of medicines in the country, though with many shortcomings. However, we are now experiencing a shortage of many essential pharmaceuticals, precipitated by the economic crisis and aggravated by gross mismanagement.
A considerable number of essential medicines and surgical supplies are not available in adequate quantities in many hospitals in the country. Furthermore, several poor-quality medicines and quality failures are reported, due to these medicines being hastily imported without proper evaluation and registration, supposedly to overcome the shortages.
In this background, instead of rectifying and improving the situation, we note that there are many regressive steps taking place at the NMRA, as listed below. This will further worsen the situation, thereby jeopardizing the safety of the patients.
1.The Honourable Minister of Health has so far failed to appoint a Professor of pharmacology, Medical Specialists already nominated by the College of Surgeons and the College of Obstetricians and Gynaecologists and a qualified pharmacist /senior lecturer or professor in Pharmacy to the Board, as per the provisions in the NMRA Act, to provide technical expertise.
2.The Pricing Committee of NMRA that is responsible for negotiations to approve retail prices of medicines is no longer functioning. We are at a loss to understand how pricing of the newly registered drugs are determined without an operational and functional pricing committee.
3.The Committee to Evaluate Quality Failures has also not been re-appointed, thereby preventing any action from being taken to improve the situation. Already there are serious allegations that some of the recent hospital deaths may have been due to drugs of poor quality.
4.Many senior experienced pharmacists who have been involved in evaluations of pharmaceuticals at the NMRA, have been suddenly transferred back to the Ministry of Health, leaving a grossly inadequate number of pharmacists to handle the pharmaceutical evaluations. This can directly affect the quality assessment of drugs prior to registration.
Taking the above stipulations into consideration, we wish to earnestly inform you that we are deeply concerned, distressed and considerably disturbed by the current developments in the NMRA, which can affect the process of registration and quality control of pharmaceuticals supplied to the country. We genuinely believe that there is a deliberate attempt by some of those in authority to sabotage the work of the NMRA. As a result, medical professionals, as well as the general public, are losing faith in the quality and safety of the drugs, which will be guaranteed to have serious deleterious effects. This is particularly so in view of the cavalier and frivolous attitude displayed by the Chairman of NMRA during his discussions with the media.
Thus, we urge you to take urgent steps without any further delay, to address the matters highlighted above and ensure proper functioning of the NMRA and thereby rebuild the confidence of the medical professionals and the general public on the medicines available in Sri Lanka.
